Full birth certificate listing both parents names, name at birth, date of birth, place of birth

2 x passport sized photographs no less than 6 months old. One of these photographs must be endorsed by the person endorsing your Form 1195 Identity Declaration

Drivers Licence

Evidence of current address

Permanent visa grant notification (if available)

Temporary visa grant notification (if available)

Name change document (if any)

Completed Form 1195 (Identity Declaration)

Australian Police Clearance (see instructions below)

You need to show a minimum of 3 documents that collectively show your birth name, date, gender, photograph, signature and current address.
